CS Form No. 100, Revised 2023 is the official application form for the Civil Service Exam (CSE-PPT). There is one version for each level, and you can download either one free from CSC. The form says it is "NOT for sale" and that reproduction is allowed. This guide walks through the form section by section, using the form itself and Examination Announcement No. 06, s. 2026 for the 14 March 2027 exam.

Key takeaways

  • Download the version for your level: Professional (PDF) or Subprofessional (PDF). Print on legal-size bond paper.
  • Do not leave any item blank. The form says to write "N/A" when an item does not apply.
  • Do not fill out the shaded portion. It is for the CSC processor only.
  • Filing in person? Leave your signature, printed name, and right thumbmark blank. You complete them in front of the CSC processor.
  • Bring with the form: four identical I.D. photos, an original and photocopy of an accepted I.D., and the ₱500.00 exam fee.

Where to get the form

Announcement No. 06 says the form is available at any CSC Regional or Field Office, or can be downloaded from csc.gov.ph. It may be printed or reproduced on legal-size bond paper.

Choose the right version. The form names the exam you are applying for, Career Service-Professional or Career Service-SubProfessional, so the version must match your level. Not sure which one? Read Professional vs Subprofessional.

The declaration: read it before you sign

Section E of the form is a declaration. By signing, you state that you personally accomplished the form, that the information is true, and that you meet all the admission requirements.

The form also spells out what happens if something is wrong:

  • If a post-verification finds information contrary to what you declared, your application is disapproved and your payment forfeited.
  • Any misrepresentation can invalidate your exam result or lead to administrative or criminal cases.
  • If your results show high answer similarity with another examinee, they are withheld while CSC investigates.

I.D. photo rules

You need four identical pictures. These are the specifications in Announcement No. 06, Section C:

RuleRequirement
SizePhilippine passport size, 4.5 cm × 3.5 cm
TakenWithin three months before filing
PrintColored, white background, good quality photo paper that does not peel
FramingFrom shoulder level up, with head and face at least 80% of the picture
FaceBare face: no eyeglasses, colored contact lenses, headdress, bandana, or accessories covering the face. Not computer enhanced
PoseFull-face view facing the camera, neutral expression, both eyes open, left and right ears showing
Name tagHand-held, about 1 inch (2.54 cm) below the chin, showing a handwritten signature over printed full name

The name tag cannot be computer-generated or inserted by computer. CSC defines hand-held as having your picture taken while you are holding the tag. Your printed name includes your given name, middle initial (if any), last name, and extension (if any).

Muslim women applicants who wear a head covering should show their forehead and both ears, but do not have to remove the covering altogether.

Accepted I.D.s

Bring the original and a photocopy of one of these. CSC says any I.D. not on the list is not accepted:

  • Driver's License or Temporary Driver's License (LTO O.R. must come with the old license; O.R. alone is not allowed), or Student Driver's Permit
  • Passport
  • PRC License
  • SSS I.D. or GSIS I.D. (UMID)
  • Voter's I.D. or Voter's Certification
  • BIR/Taxpayer's I.D. (ATM type or TIN card type with picture)

  • CSC Eligibility Card
  • Philippine Identification (PhilID) Card, including the printed ePhilID. The Digital National I.D. is subject to verification

The form says the I.D. must not be expired on filing. Announcement No. 06 adds that, as a last resort, an expired I.D. may be presented during filing if its expiry date is within 2026. Keep the same I.D. for exam day if you can: the form's rule is "NO I.D., NO EXAM."

Additional documents, if they apply to you:

  • No date of birth on your I.D.: original and photocopy of your birth certificate from the PSA or Local Civil Registry. The PSA E-Certificate is covered, subject to verification.
  • Dual citizen under R.A. 9225, or recognized as a citizen under Article IV, Section 1 of the Constitution: original and photocopy of the Identification Certificate from the Bureau of Immigration.

Check you qualify before you file

The back of the form lists the admission requirements, and Announcement No. 06 adds one more. For 14 March 2027, the key dates to check are:

  • Three-month rule: you cannot have taken the same level, in any test mode, from 14 December 2026 to 13 March 2027.
  • Already passed: you cannot retake a level you have already passed.
  • Age: you must be at least 18 on the date you file.

Read the full list in our requirements guide.

Where to file the form

CS Form No. 100 does not say how to submit it, because the mode of filing depends on the office. Announcement No. 06 tells applicants to contact the CSC Regional or Field Office where they intend to take the exam for the modes and procedures. See how to apply and the online application guide.

Filing for 14 March 2027 runs from 2 November 2026 until slots are filled or CSC announces otherwise. The exam schedule has the dates. Keep your Application Receipt: a testing center transfer request needs a copy of it.
